Be Active...Live Well Programme

Physical activity plays an important part in looking after our overall health, particularly for those at risk of or living with a long term condition.  ANGUSalive recognises that we can support healthcare partners to manage the demand on their services across Angus by being proactive when it comes to patient health.  Our Be Active…Live Well programme provides a range of activities to encourage and support an active lifestyle and we welcome referrals from healthcare professionals.

Eligibility

In order to refer to Be Active…Live Well, your patient must be inactive (less than 30 minutes of moderate physical activity per week) with, or at risk of, a controlled long term condition including but not exclusive to:

  • Musculoskeletal
  • Coronary Heart Disease and Risk Factors (inc. Hypertension or high cholesterol)
  • Stroke
  • Type 1 & 2 Diabetes
  • Cancer
  • Weight Management
  • Neurodegenerative Disease
  • Respiratory
  • Anxiety / Depression or other Mental Health
  • Chronic Fatigue
  • Under 16 years of age
  • Unstable angina
  • Unstable glucose levels
  • Resting tachycardia (HR >100bpm)
  • Uncontrolled hypertension (BP > 180/100mmhg)
  • Unstable or acute heart failure
  • New or uncontrolled arrhythmias
  • Symptomatic hypotension
  • Critical aortic stenosis
  • Febrile illness

What we provide

Be Active…Live Well ‘Active Start’ is a 12 week, free of charge, tier 2 physical activity referral programme which provides high level support and encouragement for those who are currently inactive with, or at risk of, one or more controlled long term conditions.  Participants will be internally triaged into the most suitable class for them:

  • Active Start – a circuits style class (main route and those identified as having cardiac or stroke history / at high risk of)
  • Balance Active – a strength & balance focus (for those with recent falls history / at high risk of falls)

Following completion of the 12 weeks, participants are encouraged to continue their activity through our comprehensive Stay Active programme.  Classes include seated exercise, circuits, strength & balance, gym sessions and seated yoga.  We are also expanding our delivery of Health Walks – short, free, low-level walks which can help to build confidence in walking while connecting with others.

All Active Start and Stay Active classes are delivered by our team of qualified, experienced exercise professionals who are trained in exercise referral.
